advice needed: how do you transition back once you have reached your goal?

I'm only 3 pounds away from my weightloss goal after having lost 12 pounds so far (sw: 136.3lbs, cw: 124.3lbs, gw: 121.3lbs) so I think it's time to start looking ahead a little bit so I'm somewhat prepared.

I'm just wondering if people continue to count calories forever? Or just for a transitioning period?

Do you gradually go back from deficit calorie to maintenance or just increase the whole amount from one day to the next..

I know everyone is different etc. but would really like to hear what has worked best for others :)
