How do I get back on track?

I was so pleased when I hit my target weight. But when my dad got ill and died I let it all slide. Understandably I wasn't in the place to think about diet while I was going through that. Now I've put 2/3 of the weight back on. I'm out of the worst of the grief and would like to be slim again, but I can't seem to get my act together to get back on the weighing and counting and exercise. (I've also injured my foot so can't run at the moment, which I'm gutted about.)

Do any of you have tips for getting back in the saddle after a major disruption?
