Getting sicker by the second and need help staying on track

Hey friends.

I just got my labs back from my doctor and discovered that I've got the beginnings of fatty liver disease. Before getting this news, I'd also been diagnosed with hypothyroidism, prediabetic, asthma, and sleep apnea.

I'm also in pain all of the time. Like way more than I used to be.

I'm 5'7", trans man at 250lbs and a 51" stomach circumference. I know I need to do something, and soon.

However, I've made attempts in the past and they've lasted for a month or two and then fizzled out. I had one that lasted 9 months and then Christmas came along with all of the sweets and it derailed me.

Is there a trick to stay committed, to sticking with it and seeing it through to the end? Those of you that have lost weight and kept it off, what kept you going when you really didn't want to keep going?

I need to change something and fast, or my life will be taken from me. Anything you think can help, please send it my way.
