Best diet/exercise options for me?

Hey, all. I have some questions and need some help. I've been overweight most of my life. Growing up, we didn't have a lot of money and ate cheap, bad foods a lot. It didn't help that my dad has always worked as a fast food manager and if we ate out I was told "We payed good money for that so you have to eat it all." I'm 27 now with a slew of health issues and bad eating habits. I currently weigh 240lbs. I am a male. I have bad upper back problems and a torn lower back that still hurts, even though I injured it years ago. I haven't gone to the doctor for this next issue, but most every time I eat, I have to go to the restroom almost immediately. It's never a solid situation. I have been trying keto, and while I am losing weight quickly (280 when I started), I still have the restroom issue. It's not something that's exclusive to keto, but I doubt the high fat intake helps the situation. When I moved out at 18 and started working, I rarely ate more than once a day. I still do this so I can avoid the restroom. I'm usually very hungry in the evening and eat a large dinner. I have a 40hr a week slightly physical job, but I do not do a lot of work outs. My back pain is my current excuse as well as my asthma. I want to get past that. I'm not sure what a healthy diet is. Chicken and white rice? Salads? What fruits should I eat if any? Should I consume any dairy? I also have a pretty terrible sleep schedule. I've never been good at this and don't even know where to start. Thanks for any help!
