Help -- I don't know how to get motivated to do this without exercising

I'm a 26 year old female who started having serious back problems around the beginning of August, and I'm currently seeing a physical therapist for them once a week now, though in January I'm probably going to start looking into surgical options. Currently, the issues are so bad my doctor has recommended I stop exercising, since literally everything I do seems to aggravate the issue. I can't even do gentle yoga or light walking without pain, and don't have access to a swimming pool or gym for other options. I've been drinking a lot more alcohol to deal with the chronic discomfort, and my eating habits have definitely slipped. At my lowest weight this year, I was 135, but now I'm at about 150. (Not all of that is related to my health--at 135 I was living in a European city, walking everywhere, and just living a healthier life). I look at my pictures at my lowest and healthiest weight, now, and just want to sob. I don't feel like I'll ever be able to get back to feeling that level of health and wellness.

I desperately want to get back to better habits, but in the past I've always started with exercise and then moved into dieting. My motivation just doesn't seem to be there when I'm focusing on diet alone, and I don't know how to change that. I'll start off the day eating well, but by early afternoon I'm often off the wagon. I'm already on a vegan diet, if that makes a difference.

For anyone who has lost weight without being able to exercise, how did you do it? Where would you recommend starting?
