How do I start?

I don't like how I look. I haven't for a long time. When I was back in high school I dieted after a knee surgery by using CICO for a few months and it worked fine. But now every time I try to make even one small step toward being healthier I get discouraged and take 3 steps back. I'm sure a lot of people here struggle with depression and lack of motivation and the whole shebang. Where do I start? How do I get over my overwhelming anxiety at the thought of exercising, or make sustainable healthier food choices? I keep wanting to try and start, and I can never get it to stick. I'm starting to feel like I'm destined to be lazy and fat and ugly for the rest of my life.
