M 28 287lbs 5’11 need help!

I need opinions on what to do here. I’m a very bad yo-yo dieter and I am tired of it. My biggest was 305lbs. I work a night shift in a stressful job.

This is my issue, when I wake up in the afternoon I have zero appetite. So I take this as an opportunity to fast. If I eat at all I get bloated and feel sick especially in the afternoon. The only time I can eat is when I get home in the morning and I am prone to bingeing, especially on carbs. Carb cravings being a direct result of the cortisol spikes from stress. This makes it even harder to exercise because I feel so sapped of any energy. I’m trying to do cardio 4-5 times a week with weight training mixed in. Even if I am consistent with my workouts I don’t lose weight. I know it’s because I’m bingeing right before I go to bed.

I did keto a year back and had great results but I stopped because I started to have moments of elevated heart rate. I went back to eating a little more carbs and it went away but then the bingeing came back slowly but surely.

I’m sure this all comes down to me needing to see a doctor. But I was hoping for a little insight and maybe tips from someone in a similar situation.
