Tips for someone with disabilities?

I'm going to try and keep this as short as possible without leaving anything out but I'm sorry if it runs long regardless.
So the basics: I am 18 years old, FTM, 5'7", and hover around 160lbs. I have POTS, EDS, and Fibromyalgia (along with a few other mental and neurological conditions I feel don't apply to this setting.) I am currently not on any medications though I may start my testosterone injects again when things settle down politically in my area.

I know that realistically 160 is not horrible but I also know it's not great. My family has a history of heart disease and a common trend of obesity so I would like to try and keep my weight in a lower, but, healthy range.
In the past, I could not keep the weight I had very well and that combined with an eating disorder made me never be heavier than 120lbs for many years. I was still 5'7" at the time as well. But when I finally started to recover and I grew a bit older I suddenly started gaining very rapidly and am at where I am now. Exercise is unfortunately somewhat hard to be due to what's easiest described as me having 'loose joints' as they pop out fully or partially somewhat easy and semi-frequently. My fibro and POTS do not help this and my heart rate regularly can reach 135-140 simply from standing.

What I need help with: If possible I would love some advice on some exercises I can do to help me become more active and burn calories without hurting myself or passing out due to medical conditions. I'd especially love low-intensity ones I can do for long periods or over the course of the days or even if there are things I can do without rapid up and down motions. Or even ideas on ways I may be able to diet without relapsing into starvation and purging. (diets that include just healthy eating/cutting certain things out may work better for me than calorie counting but I have no idea how effective those are)

And yes, I have been in contact with my doctors but I have been unable to see them in person for months and most don't share the same concerns I have about my weight. I also know this is a complex issue and there may not be a lot of answers but it's a big community so any advice no matter how small is greatly appreciated !
