question about body types

forgive me if this doesn't belong in this subreddit but I'm not entirely sure where else it would go. i was thinking people who are experienced with losing weight and how it affects the body would be able to give me some answers. so, i have a friend (a real friend, this isn't a "im asking for a friend lol" situation) who has a bunch of illnesses and one of them causes him to be underweight, the entire time i've known and been friends with him he's been underweight, like around 17 bmi. he has no definition to his body and he's just a walking stick, but he's actually pretty active, maybe not like consistently the way people who go to the gym do but he hikes and climbs and all that shit. and recently i've been watching a lot of movies thanks to the free time quarantine has been giving me, and specifically i've watched the machinist with christian bale and hunger with michael fassbender, and in both of those movies they're underweight at that level too. but compared to my friend, they both have this weird definition to their bodies, like you can see that what little meat they have on their bodies is solid, there's curves on their biceps and shit, and i'm wondering if the difference between them is exercise, or bone structure?
